Total Wellbeing Luton: Active Luton

If you’re feeling lonely, isolated or low in mood, are living with a long term health condition or struggling with a personal situation or carer responsibilities, our Social Prescription programme could help you.

Social Prescription is designed to help people take control back of their lives and put people in touch with local services that can help them in a wide variety of ways.

You can be referred to the Social Prescription Link Worker by your GP or a Healthcare professional or you can self-refer.

You can receive up to 12 weeks of funded access to  particular activities within the community, for example physical activity, social/support groups, creative groups, and advice from specialist services.

Contact your GP and ask for a referral to your Social Prescription Link Worker

Call 0300 555 4152 to self-refer
