This video provides clarity around how to support people to manage their own wellbeing. Knowledge, skills and confidence are key to people’s active involvement in their own well-being. Practitioners in primary care and social prescribing talked to us about the value of using the Patient Activation Measure to assess patients’ levels of knowledge, skills and confidence, enabling them to provide the support that is right for the patient and see the impact of their work is having with their patients.
