Social Prescribing Link Workers

A Social Prescribing Link Worker can offer patients a Personalised Care Approach and offer time and local knowledge to help patients identify what matters to them. They can connect patients with community groups and agencies for practical and emotional support.

Whether you employ a Link worker directly or commission from a local service, their role is to collaborate with local partners to support community groups to be accessible and sustainable and help people to start new groups.

They can take a lead on supporting patients with:

•LTCs and complex needs
•Mental health needs
•Patients who are lonely or isolated
•Patients experience social problems that affects wellbeing.

Your PCN may consider employing a Care Coordinator, Link Worker and Health and Wellbeing Coach as they all focus on delivering personalised care.
