Bedfordshire Dementia Roadmap: identifying and accessing services locally

The main audience is staff working in primary care, including GPs, nurses, dementia navigators and practice managers. The roadmap will also be of benefit to other professionals, people worried about memory problems, people living with dementia and their carers and families.

The Dementia Roadmap is a web based platform that provides high quality information about the dementia journey alongside local information about services, support groups and care pathways to assist primary care staff to more effectively support people with dementia, their families and carers.

It was developed in a partnership between the Royal College of General Practitioners and the Department of Health. There is a subscription charge, however The Bedfordshire Dementia Roadmap has been sponsored by “Unforgettable” Live Better with Dementia and hosted by Dementia Pathways.
