Clinical Pharmacists
Clinical Pharmacists are highly qualified experts and can support MDT working by clinically assessing and treating patients using their expert knowledge of medicines for specific disease areas and long-term conditions. They can be effectively utilised to offer prescribing and medication review to patients with complex health needs/multiple co-morbidities and support patients to develop self-care skills.
Clinical Pharmacists can play a critical role in developing close working relationships with Pharmacists in other settings connected to the PCN and can develop to take on Leadership roles.
With additional training, Clinical Pharmacists can take on additional areas of assessment & review.
